The CRESEMBA Patient Savings Program^a is for eligible adult patients who have commercial prescription insurance. The program parameters are as follows:

• You must have a valid prescription, meet the eligibility requirements, and present the Savings Card to your pharmacist
• You may be eligible to pay as little as $25 per prescription
• You are covered up to a maximum savings of $4000 per calendar year
• There are no income requirements

Obtaining the Savings Card

You must present the Savings Card at the pharmacy when you fill your prescription. There are 2 ways for you to receive the Savings Card:
