Patient Enrollment Process

After the patient has been prescribed PADCEV® (enfortumab vedotin-ejfv), the patient can be enrolled in PADCEV Support Solutions one of the following three ways:

 

  1. Enroll by Fax
  2. Enroll via Prescriber Portala
    • Log in to the Prescriber Portal to enroll the patient by completing the Patient Enrollment Form
  3. Enroll by Phone
    • Contact PADCEV Support Solutions to enroll over the phone
    • Call 1-888-402-0627, Monday–Friday, 8:30 am–8:00 pm ET
