Patient Enrollment Process

After the patient has been prescribed PADCEV^® (enfortumab vedotin-ejfv), the patient can be enrolled in PADCEV Support Solutions one of the following three ways:

Enroll by Fax
- Complete the Patient Enrollment Form, including the patient's and healthcare provider's signatures, and fax it to 1-877-747-6843
Enroll via Prescriber Portal^a
- Log in to the Prescriber Portal to enroll the patient by completing the Patient Enrollment Form
Enroll by Phone
- Contact PADCEV Support Solutions to enroll over the phone
- Call 1-888-402-0627, Monday–Friday, 8:30 am–8:00 pm ET
